When someone is re-querying you with a manuscript which has been revised based on another agent's feedback (You indicate in your FAQ that you are open to one re-query), is it helpful, or considered bad form, to identify the agent who gave the feedback? Is it relevant if the agent is very highly regarded in the industry? Thank you.

If I’m the agent who’s given feedback tell me. If I’m not instead focus on what you revised.

If you named the agent, my first question would be “And why haven’t they taken this on if you did their revisions?” and that’s not what you want me to be thinking.
